Health History Form

As a new University student, you must verify certain health requirements before you enroll at UConn. All students must submit information about their measles and rubella immunizations, as well as tuberculosis tests.

Please inform the University of your medical history by completing the Health History Form that is included in the Orientation packet you will receive in the mail.

This confidential record will help us better serve your health needs throughout your academic career at UConn. Your completed form is due by January 1.

Meningitis Immunization

If you intend to live on campus, Connecticut State Law requires that any student living in University-owned housing be vaccinated against meningitis as a condition for living in a University's residence halls or apartments.

You and your physician need to certify that you have been immunized with the meningococcal vaccine within the past five years.

A Meningitis Immunization section is part of the Health History Form that is included in your Orientation packet. Students who submit their Health History Form after January 1 jeopardize their access to UConn housing.
